Surgical Observations in the Aged, 60 Years and Over




Abstract
An analysis was made of the records of 453 patients more than 6oyears of age, who had admitted in the general surgical department at the Yonsei University medical center during period of 2 years from Jan. 1978 to Dec. 1979. The results were as follow;
1) Hospitalization and the operation rate had to be increased accordings to increasing number of the aged, 60 years and over.
2) There were 289 men and 164 women and the male to female ratio of 1:0.56. The operation rate was 83.0%.
3) Admitted patients via E.R. were 135(35.9%) and their percentage was still high and it also effects postoperative prognosis. The operations performed emergently were 103(27.3%) and those performed electively were 273 cases(72.6%),
4) According to the disease analysis, upper G-I tract disease were 179 cases(47.6%), of which stomach cancer was the most common one and the lower G-I tract disease were 131 cases(34.8%), of which acute appendicitis was. the most common one. Two hunderd and sixty three patient were nonmalignant group and the malignant group were 190 cases and their operation rate was 64.8%. Those disclosed that the prevalency of malignant disease than other age group. The malignant disease were stomach ca., colorectal ca., and biliary Ca. in decreasing order.
5) Coexistent diseases were noted in 162 patients(42%) and 84 patients had more than two .coexistant diseases. The most common problems were pulmonary diseases those influenced to the postoperative complication and mortality significantly.
6) The postoperative complication developed in 149 patients(33%) and those mainly affected to pulmonary system. Those frequently reported in foreigner¢¥s report?such as renal and thromboembolism were rare.
7) Postoperative mortality rate was encountered only 3.2% in this analysis which is lesser than other reports due to increased elective operations, narrow range of age and relatively frequent cases of moribund discharges. The mortality rate in emergency operation was 5.8% and that was as three times higher as elective operation(2.1%).
